Tips for Using SurveyMonkey:
A template is available on the Dashboard named “NCCSD Template.” Change the title of your new survey in the following manner: [The name of your state or committee] – [Survey title]. SurveyMonkey allows for only 10 questions. Your name and email address are also preferred; but if you need more than seven questions, please keep “Your state.” This will allow users to know who created the survey.
